Boarding Contract

Body Agreement

This is a contract between Howllywood Boarding and Grooming (herein after called HB&G) and the pet owner whose signature appears below (herein after called "owner")

1. Owner agrees to pay the rate for boarding in effect on the date pet(s) is checked into HB&G (as agreed upon)

2. Owner agrees to pay all costs and charges for special services requested such as grooming and medication fees. As well as all veterinary costs for the pet(s) during the time said pet(s) are in the care of HB&G

3. Owner further agrees that the pet(s) shall not leave the HB&G property until all charges are paid to HB&G by the owner

4. By signing this contract and leaving the pet(s) with HB&G the owner certified to the accuracy of all information about said pet(s) in the contract.

5. HB&G shall exercise reasonable care for the pet delivered by the owner to HB&G for boarding. It is expressly agreed by the owner and HWB&G that the HWB&G's liability shall in no event be the lesser of the current value of the pet of the same species or the sum of $500.00 per animal boarded. The owner further agrees to be solely responsible for any and all acts of behavior of said pets while it is in the care of HWB&G.

6. I understand that the kennel and playtime enclosures are constructed with six foot chain link fence with additional electric wire within the fence perimeter. I am satisfied that the height of the fence is sufficient to contain my pet(s). I agree not to hold HB&G responsible if my dogs jumps or climbs the fence or in anyway escapes the secured enclosure.

7. I understand that there is always a risk when choosing to put my dog out with others dogs in the play yard and agree not to hold HB&G responsible for any injuries resulting in vet fees that could be incurred due to altercations between the dogs or branches of fences in the play yard.

8. I understand that dogs play with each other and will not hold HB&G responsible for any collars or harnesses that may be chewed during playtime. HB&G also are not responsible for any bedding, blankets or bowls that may be chewed during their stay.

9. Owner specifically represents that he/she is the sole owner of the pet, free and clear of all liens.

10. All charges incurred by the owner must be paid in full at time of pickup. The owner hereby agrees that in the event that all charges are not paid in full at time of pick up that the pet will not be released to the owner until such time that payment in made in full.

11. In the event of a medical emergency, the owner instructs HB&G to 1. Attempt to contact me or my emergency contacts person noted within the contents of the contract, 2. seek immediate medical care with my knowledge that I will be liable for all costs incurred. I do give HB&G permission to use their beset discretion for when medical care is necessary for my pet(s). If it is a immediate medical emergency your pet will be taken across the street to Minter Vet for fast care. HB&G is not to be held responsible.

12. This contract contains the entire agreement between the parties. All terms and conditions are binding upon signature.

13. Any controversy or claim arising out of, or relating to this contract; or breach thereof, or as a result of any claim or controversy involving alleged negligence by any party of this contract, shall be settled by arbitration in accordance with the rules of the American Arbitration Association and judgment upon the award rendered by an arbitrator may be entered in any court having jurisdiction thereof. The arbitrator shall, as a part of his/her award, determine and award to the prevailing party part of the costs of such arbitration and reasonable attorneys fees of the prevailing party.

14. All animals boarded on these premises have current documented vaccinations, however HB&G will not be held responsible in the event that a pet contracts Bordatella or other distemper related diseases while at HB&G. Required vaccines are Rabies, all boosters of DHPP combo and Bordetella

15. This contract is to be considered in effect until canceled in writing by either party.
